WHAT INFORMATION DO WE COLLECT?  

Personal information you disclose to us The personal information that we collect depends on the context of your interactions with us and the Sites or App, the choices you make and the products and features you use. The personal information we collect can include the following:

Name and Contact Data. We may collect your name, phone number and email address depending on the service you use.

Credentials. The application requires a One Time Verification Code sent via Auth0, our identity provider, which is stored by the application. All personal information that you provide to us must be true, complete and accurate, and you must notify us of any changes to such personal information.  

Information automatically collected We automatically collect certain information when you visit, use or navigate the Sites or Apps. This information does not reveal your specific identity (like your name or contact information) but may include device and usage information, such as your IP address, browser and device characteristics, operating system, language preferences, referring URLs, device name, country, location, information about how and when you use our Sites or Apps and other technical information. This information is primarily needed to maintain the security and operation of our Sites or Apps, and for our internal analytics and reporting purposes. 
Like many businesses, we also collect information through cookies and similar technologies.   

Information collected through our Apps If you use our Apps, we may also collect the following information:   

Your FormScore. This is the score of your mental health and wellbeing using the FormScore scale - which is a score of between 1 - 10. We will also collect the 'drivers' of that score if you enter them. Drivers include (among others) work, nutrition, exercise and sleep.

Your Messages.  Messages are stored on our servers and are encrypted at rest and in transit, so we are unable to view the content of your messages.    

Your Connections. To help you organise how you communicate with others, we may help you identify your contacts who also use Form, and you can create, join, or get added to communities or groups, and such groups and lists get associated with your account information. You give your groups a name. You can decide to provide a group profile picture or description.

Mobile Device Access. We may request access or permission to certain features from your mobile device, including your mobile device’s contacts, camera, microphone, reminders, social media accounts, calendar, storage, and other features. If you wish to change our access or permissions, you may do so in your device’s settings.    

Mobile Device Data. We may automatically collect device information (such as your mobile device ID, model and manufacturer), operating system, version information and IP address.    

Push Notifications. We may request to send you push notifications regarding your account or the mobile application. If you wish to opt-out from receiving these types of communications, you may turn them off in your device’s settings.   

Aggregated and Anonymised Data We also collect, use and share Aggregated and Anonymised Data such as statistical or demographic data for any purpose. Aggregated Data may be derived from your personal data but is not considered personal data in law as this data does not directly or indirectly reveal your identity. For example, we may aggregate your Usage Data to calculate the percentage of users accessing a specific website feature. However, if we combine or connect Aggregated Data with your personal data so that it can directly or indirectly identify you, we treat the combined data as personal data which will be used in accordance with this Privacy Policy.

CONTROLS FOR DO-NOT-TRACK FEATURES  

Most web browsers and some mobile operating systems and mobile applications include a Do-Not-Track (“DNT”) feature or setting you can activate to signal your privacy preference not to have data about your online browsing activities monitored and collected. No uniform technology standard for recognizing and implementing DNT signals has been finalized. As such, we do not currently respond to DNT browser signals or any other mechanism that automatically communicates your choice not to be tracked online.
