FormScore v2 is available both on the App Store and Google Play.

System Requirements:

• iOS 10.0 or later

• Android 4.0 or later

To download the latest version of the app search for ‘FormScore’ in App Store or Google Play.

Limitations:

The sign up is based on a valid mobile phone number, as opposed to an email address. This means that the FormScore app can only be downloaded on smart phones but not SIM-free iPads or Tablet devices. We apologise for the inconvenience.
