How it started
We started Form as a simple way to track our mental wellbeing on a day-to-day basis using a score, out of ten. From there, FormScore has evolved to be much more than a mood tracker. The App connects us with friends, family and colleagues to help support each other when we’re feeling low and celebrate when we’re riding high.
Why now?
Now more than ever, we need to be able to keep track of our mental health and support those around us.
The stigma of mental ill-health often makes it hard for us to share how we are feeling. The effects of the pandemic mean that 100% of us will know what it is like to struggle with our mental health to some degree or another. Yet we are required to be physically distant from those who might be able to offer support.
How?
FormScore provides a new and simple way of communicating with a score out of ten. Then the app facilitates connection between those we trust in order to make us aware of when friends might need a check in and also lets them know when we might need some support.
We have ambitious plans to change the way we think and act about mental health.
We all have mental health. We all have Form.

What do you think of when I say mental health?
Struggle? Illness? The story of someone else?
Depression? Anxiety? PTSD?
Mental health? No, mental health doesn’t relate to you or me.
Except it does.
Mental health belongs to all of us.
Stigma calls it illness and weakness and failure
but really it’s about promoting positive patterns of behaviour.
Investment in people, the spark of human connection
Empathy, acceptance, beauty in imperfection. →
Mental health needs a name change.
Mental health needs a game-change.
So let’s talk about FORM instead;
a number to communicate what’s happening in our heads.
One, two, three, four, five -
a simple score to help us thrive,

let’s check in with each other every now and then.
Mental health is an issue that will define our time.
Together we must challenge the dominant paradigm;
this belief that wellbeing exists outside of the norm.
