Navigate to the FormScore app/plug-in within Microsoft Teams

In the chat/BOT window, type Help

Hit the Send button to see all options available

Type Set Score, hit Send to log your score

From the drop-down list, click on the score that represent your Form

• 10 being a lifetime high, 6 being moderate form and 1 the lowest form for you

Then choose your Drivers of Form

• Pro tip: you can select more than one drivers from each category
