FormScore, Wellbeing, Effectiveness, Commitment what do they mean?

FormScore

Your FormScore is a measure of your readiness to perform broken down into:

  • Wellbeing

  • Effectiveness

  • Commitment to your organisation

During the poll, move the slides to score each of the categories out of 10. Your FormScore is the average of the three. Note, we never share your individual data with your manager.


Wellbeing

Answering the question ‘How are you today?

During the poll move the slider to record your wellbeing score where 10 is your lifetime peak, 6 is average and 1 is the lowest possible for you.

Reflect on the factors driving your wellness: sleep, exercise, connections, finances, stress, nutrition, mental and physical health. Don't overthink it. Just go with your gut feeling.


Effectiveness

Answering the question ‘How effective do you feel today?

Think about your energy, motivation, workload, work, stress, resources and support available and give yourself a score out of 10.


Commitment.

Answering the question ‘How committed do you feel?

Let's face it, we all have days when we feel more committed than other days. Reflect on the factors that drive your commitment: belonging autonomy, fulfilment, fairness and feedback among others. What's your commitment score out of 10?
